What is a Support Coordinator?

Navigating the realm of healthcare and social services can be daunting, especially for individuals with disabilities or additional needs. A Support Coordinator helps bridge the gap between formal and informal supports; mainstream and community supports. Support Coordinators can act as a guiding hand to find suitable and available providers and help individuals achieve goals and live life to it’s potential.

The role of a Support Coordinator is incredibly broad, and no two days are the same. A Support Coordinator will work alongside an NDIS participant, to understand their abilities and how their disability affects their lives. This collaborative relationship allows Support Coordinators to find suitable providers and services, build networks and tailor support to individual circumstances. We understand that each disability is unique, and its impact on an individual’s life can vary dramatically from person to person. That’s why we will work directly with the participant and their supports, to empower and advocate for them, and assist them to make informed decisions with choice and control. 

Support Coordinators will monitor and review plans to ensure they are effective, relevant and remain suitable. This means reviewing participant goals, and monitoring funds and spending, to ensure they are utilizing their funds effectively and getting the most out of their plan as possible.

With knowledge of their local area, Support Coordinators collaborate with providers and allied health services, and work as a team to ensure the participant is receiving the best possible support to reach their goals. They will liaise with these parties’ and review reports and documentation and provide evidence to the Agency if required.

Support Coordinators will identify and evaluate any barriers or challenges that may arise, and make any necessary adjustments to the care plan. This includes compiling and generating any documents that need to be sent to the Agency for Plan Reviews or Variations, or Change of Circumstances. Coordinators will liaise with the Agency on your behalf and complete any paperwork or behind the scenes documents as required.

Other than the technical and official duties of a Support Coordinator, the role also offers the participant another form of support and a person to turn to if they need help or guidance. Although the role is focused on engaging supports and monitoring plans, its overarching goal is to empower individuals, promote inclusion and give participants the opportunity to have control over their NDIS plan and ultimately their lives.
